| Your Appointment: |
On your first visit to our office, please be prepared to complete a patient information form and medical history.
Please bring to your appointment, all insurance cards. If your HMO requires a referral form, please also bring this to your appointment.
If your studies have been performed at an institution other than Lancaster General Hospital, you may be asked to bring them with you. |

| When to call us: |
Our office hours are Monday through Friday 8:00 am – 4:00 pm. If you are calling about a matter that does not require immediate assistance, please call during office hours.
If you require immediate assistance related to a surgical problem, we are available 24 hours a day, 7 days a week through our answering service.
Medical problems and prescription refills are usually dealt with by calling your cardiologist or family physician.
